I wish to offer my diving students and clients access to membership in the DiveAssure Association , and consequently offer them eligibility for diving accident and dive & travel coverage programs.

I hereby authorize DiveAssure to issue a StartDive certificate activating free membership and free DCS coverage to my Entry Level, Open Water Scuba and freediving students that I certify, covering them for Decompression Sickness for the duration of their certification process (up to 6 months).

I understand that DiveAssure adheres to strict privacy guidelines and all information provided by me is strictly confidential and will not be used for any purpose other than diving and traveling coverage nor will it be disclosed to any other party.

I will be entitled to referral fees for all coverage plans referred by me to DiveAssure.

I understand that in the event that there shall occur at any time a change in applicable law or regulation of any governmental body, or in the interpretation or implementation of any such applicable law or regulation, or any applicable material change in legal or underwriting or rating agreements pertaining to DiveAssure which materially and adversely affects the cost to DiveAssure of providing the service or services it is obligated to provide under this Agreement, DiveAssure will renegotiate the terms of this Agreement.
